Once you complete your order, you will receive a pdf file with a link to a survey. In that survey, you will need to tell us how much money you originally spent on your investments on Celsius and how much property you received in your distribution. This information is necessary to calculate your capital gains/losses (we cannot prepare your report without this information!) If you'd like to see an example of what this survey looks like before purchasing, we have posted an example image of the report.
 
Note that, while the report you receive will contain all the information we describe above, you are responsible for reporting these amounts on the appropriate tax return. We are not a tax filing or return preparation service.
